Boarding Student Permissions (BOARDERS)

Students are not approved to leave campus (for other than supervised activities) unless a parent has given permissions as indicated below.

Please read the following and indicate your permissions by checking one box per section.

Boarding Student Permissions

Student may NOT leave campus on foot.

Student may leave campus on foot with no restrictions.

Student may leave campus on foot, but only with another CH-CH student, faculty or staff member.

Student may NOT use public/private transportation.

Student may use public/private transportation with no restrictions.

Student may only use public/private transportation with anther CH-CH student, faculty or staff member.

Student may NOT leave campus by car.

Student may leave by car with parents of other CH-CH students.

Student may be driven by other CH-CH student IF driver has permission to transport.
