Students are not approved to leave campus (for other than supervised activities) unless a parent has given permissions as indicated below.

Destinations approved for overnight visits (other than with a parent with legal custody or guardian) must have a supervising adult (over 30 years of age or approved by the grade level Program Director).
Please provide: Supervising adult’s name, address, city, state, zip, and phone number.
Parent's initials below indicate agreement to the content being submitted on this form.
After pressing "Submit" you may see a message that states:

 "Inquiry Saved. Thank You." or "The Form has been successfully submitted."
